FAQs — GENERAL PRACTICE

  • Path Health is a private-pay practice. We do not participate in any insurance plans, and we are unable to submit claims or superbills for reimbursement on behalf of clients. Superbills are, however, available on request and provided to the client directly. We are also unable to provide services to individuals covered under Medicare or Medicaid.

    This model is intentional. Insurance-based care limits the time, the testing, and the treatments a physician can offer. Operating outside of that system means we can order the labs that are clinically indicated for your situation, spend the time your case actually requires, and recommend treatments based on evidence — not on what a coverage policy approves.

    We do accept HSA and FSA payment, which allows many clients to use pre-tax dollars to invest in their care. All major credit and debit cards are also accepted. Limited financing options are also available.

  • No. Path Health is a specialized practice focused on hormone optimization, metabolic health, and Lifestyle Medicine. We are not a primary care practice and do not provide general health management, routine physical exams, or acute care.

    It is important that you continue seeing your primary care provider for routine care, preventive screenings, and management of any other health conditions. We work alongside your PCP, not in place of them, and we encourage you to keep your PCP informed of the care you are receiving at Path Health.

    There may be occasions we refer you back to your primary care provider or to another specialist for services such as mammograms, pap smears, or additional diagnostic workup. That kind of collaboration is part of how comprehensive care should work.

  • EMBARK is the starting point for all new clients at Path Health. It is a comprehensive, physician-led evaluation designed to give us the full clinical picture before making any recommendations. Here is how the process works:

    • Lab Collection:  After booking, your first step is completing your advanced wellness lab draw. These panels go well beyond a standard CBC, evaluating hormonal balance, thyroid function, metabolic markers, inflammatory indicators, micronutrient levels, and other diagnostics that routine physicals commonly miss.

    • Pre-Visit Intake:  While your labs are processing, you will complete a detailed health history and lifestyle questionnaire through your Patient Portal. This is carefully reviewed before your appointment, so your time with the physician is focused on answers, not background.

    • 60-Minute Consultation:  Once your labs have resulted, you will schedule your consultation available in person at our Buckhead, Atlanta location or via TeleHealth for clients in Georgia and Texas (with additional states coming soon). Dr. Herbert walks through your labs, body composition data, and intake in full.

    • Your Personalized Wellness Roadmap:  At the close of your consultation, your roadmap is delivered. It is a clear, clinical plan explaining what is driving your symptoms and what your body needs. For clients who are the right fit, an invitation to join one of our exclusive Wellness Programs is extended at this time.

    EMBARK also includes body composition analysis using InBody clinical technology, evaluating lean muscle mass, body fat percentage, visceral fat levels, and metabolic baseline. This gives important additional data that a standard scale simply cannot provide.

  • We accept all major credit and debit cards (Visa, Mastercard, American Express, Discover) as well as HSA (Health Savings Account) and FSA (Flexible Spending Account) cards.

    We do not accept insurance, Medicare, or Medicaid. All fees are collected at the time of service or at the time of scheduling, depending on appointment type. Pricing is always discussed transparently. There are no hidden fees or unexpected charges.

  • We ask that all cancellations and reschedules be made at least 48 hours before your scheduled appointment. Here is how the policy works:

    • Cancellation with 48 hours or more notice:  Full refund of your appointment fee, minus a 3% processing fee.

    • Cancellation with less than 48 hours notice:  50% of your appointment fee is forfeited. The remaining 50% will be refunded.

    • Rescheduling with less than 48 hours notice:  A rescheduling fee will be charged to the card on file. Full details are provided at the time of booking.

    • No-show (no notice given):  50% of the appointment fee is forfeited. No further refund will be issued.

    • Late arrivals:  If you arrive 15 or more minutes late without prior notice, your appointment may need to be rescheduled. Please contact us as early as possible at 888-720-PATH if you know you will be running late.

    • Lab fees and EMBARK fee once labs are drawn:  Once your lab work has been collected, the full EMBARK fee becomes non-refundable. As the ordering physician assumes clinical responsibility for reviewing and interpreting your results, regardless of whether the consultation is completed. That responsibility begins the moment labs are drawn and so does the fee.

    These policies exist to protect both your time and the availability of appointments for all clients. In the event of a genuine emergency, exceptions may be considered at Path Health’s discretion. Complete policy details are also available in your Patient Portal at the time of booking.

  • No. While Path Health's messaging is primarily focused on the unique hormonal and metabolic challenges that women over 40 face, we also treat men. Hormone decline, metabolic dysfunction, and the physiologic changes that come with aging are not exclusive to women and men deserve the same level of personalized, physician-led care.

    Men navigating testosterone decline, metabolic resistance, changes in body composition, fatigue, cognitive shifts, or weight gain that is no longer responding to their previous approach will find the same comprehensive EMBARK evaluation and evidence-based treatment framework available to them at Path Health. The science of hormonal and metabolic optimization applies equally, and we bring the same clinical rigor to every client regardless of gender.

FAQs — HORMONE OPTIMIZATION

  • Bioidentical Hormone Replacement Therapy (BHRT) uses hormones that are structurally identical to those produced naturally by your body. They are derived from plant sources and are designed to interact with your hormone receptors the same way your own hormones do.

    BHRT includes both FDA-approved commercially available medications and compounded formulations that are customized to your specific dose and delivery needs. Compounded BHRT allows for precise individualization, adjusting the type, dose, and delivery method to match your body's requirements, something that is not always possible with fixed, mass-produced formulations.

    Conventional (synthetic) hormone therapy uses artificially manufactured compounds that interact differently with hormone receptors. These are primarily found in FDA-approved commercially available medications. While effective for many women, synthetic formulations generally carry a different risk profile than bioidentical hormones.

    At Path Health, the decision between BHRT and conventional therapy is made collaboratively with Dr. Herbert based on your clinical data, medical history, and individual goals, never by protocol.

  • Yes, when appropriately prescribed and monitored. The fear surrounding hormone therapy largely stems from a 2002 Women's Health Initiative (WHI) study that was later found to be significantly flawed in its methodology and its applicability to healthy, younger menopausal women. Current evidence from leading bodies, including The Menopause Society, is clear: for most women under 60 or within 10 years of menopause onset, hormone therapy has a favorable benefit-to-risk profile.

    Here is what the current evidence supports:

    • Hormone therapy is the most effective treatment for hot flashes, night sweats, sleep disruption, vaginal atrophy, and other menopausal symptoms.

    • Topical (transdermal) estrogen does not carry the same risk of blood clots or stroke as oral estrogen.

    • Properly dosed hormone therapy does not increase breast cancer risk above the baseline risk for the general population.

    • Hormone therapy is associated with reduced risk of osteoporosis, cardiovascular disease, type 2 diabetes, cognitive decline, and overall mortality when initiated at the appropriate time.

    That said, hormone therapy is not appropriate for every woman. We conduct a thorough evaluation of your personal and family medical history, contraindications, and risk factors before any recommendation is made. Every decision is made collaboratively with your full understanding of the risks, benefits, and alternatives.

  • Hormone therapy is not appropriate for all women. You may not be a candidate if you have any of the following:

    • Unexplained vaginal bleeding after menopause

    • Current or prior history of breast cancer or other estrogen- or progesterone-sensitive cancers

    • Current or prior history of liver disease (including hepatitis, NAFLD, or cirrhosis)

    • Prior history of coronary heart disease, stroke, or heart attack

    • Current or prior history of blood clots or pulmonary embolism

    • Current pregnancy or breastfeeding

    This is not an exhaustive list, and having one of these conditions does not automatically disqualify you from all forms of hormone support. Dr. Herbert evaluates each client individually. If standard hormone therapy is not appropriate for your situation, alternative approaches will be discussed during your consultation.

  • Path Health offers a comprehensive range of hormone therapy options including estrogen, progesterone, testosterone, and DHEA. Available delivery forms include:

    • Topical creams and gels:  transdermal application for steady hormone absorption without the clot and stroke risk associated with oral estrogen

    • Oral capsules:  convenient for certain hormone formulations, particularly progesterone

    • Vaginal preparations:  targeted local therapy for symptoms including dryness, atrophy, painful intercourse, and recurrent UTIs

    • Injections:  for clients whose clinical picture and preferences make injectable therapy the most effective option

    • Hormone pellets:  small, subcutaneously implanted pellets that deliver a steady release of hormones over several months

    The right delivery method for you will be determined based on your EMBARK results, your medical history, your lifestyle, and your personal preferences. You will be walked through all appropriate options during your consultation so you can make a fully informed decision.

  • Many women notice initial improvements within two to four weeks of starting hormone therapy. The full therapeutic effect, however, typically develops over 8 to 12 weeks. Individual response varies depending on your starting hormone levels, the specific therapy prescribed, and the degree of hormonal fluctuation you are experiencing.

    It is also realistic to understand that hormone therapy aims to significantly reduce symptoms. A reasonable goal is 80% improvement rather than complete elimination, particularly for women in active perimenopause, where hormone levels can shift considerably over time. Dose adjustments may be needed as your body and your hormones change, and ongoing communication with Dr. Herbert through your Wellness Program is an important part of the process.

  • Perimenopause is the transitional phase leading up to menopause, often beginning in a woman's late 30s or early 40s, during which estrogen and progesterone levels fluctuate and gradually decline. This phase can last anywhere from a few years to over a decade, and it is during this time that most women first experience symptoms: irregular cycles, brain fog, disrupted sleep, weight changes, mood shifts, hot flashes, and reduced libido.

    Importantly, perimenopause is a clinical diagnosis, meaning it is identified based on your symptoms and medical history, not solely on lab results. Hormone levels during perimenopause fluctuate significantly, so a single "normal" lab draw does not rule out perimenopause.

    Menopause is defined as 12 consecutive months without a menstrual period. It marks the end of ovarian hormone production and typically occurs between ages 45 and 55, with the average age being 51 in the United States. The years following menopause, postmenopause, carry their own distinct health considerations, including accelerated bone loss, cardiovascular risk changes, and continued hormonal symptoms.

FAQs — METABOLIC WEIGHT LOSS

  • Weight resistance in your 40s and 50s is not a motivation problem; it is a physiology problem. As estrogen and progesterone decline, your body's insulin sensitivity decreases, cortisol becomes more disruptive, lean muscle mass begins to erode, and fat redistribution, particularly visceral, abdominal fat, accelerates. The strategies that worked in your 30s were designed for a different hormonal environment. They are not failing because you are doing them wrong.

    Path Health's approach to metabolic weight loss begins with the EMBARK evaluation: comprehensive labs and body composition analysis that identify exactly what is happening metabolically before we recommend any intervention. From there, we build a plan that addresses the root drivers, not just calories in and calories out.

    That plan may include hormone optimization to restore the metabolic environment, targeted nutrition and movement strategies grounded in Lifestyle Medicine, and, when clinically appropriate, physician-supervised GLP-1 therapy. Every element works together because sustainable results require addressing the whole picture.

  • GLP-1 receptor agonists are a class of medications that mimic a naturally occurring hormone called glucagon-like peptide-1. This hormone plays a central role in regulating appetite, blood sugar, and insulin secretion. GLP-1 medications work by slowing gastric emptying (so you feel full longer), reducing appetite signals in the brain, improving insulin sensitivity, and supporting sustainable fat loss, particularly visceral fat.

    The most commonly used GLP-1 medications in weight management are:

    • Semaglutide:  a weekly injectable or oral medication with strong clinical evidence for both weight loss and cardiovascular protection.

    • Tirzepatide:  a dual GLP-1/GIP receptor agonist that has demonstrated even greater metabolic and weight loss outcomes in clinical trials, including significant improvements in insulin resistance and body composition.

    At Path Health, GLP-1 therapy is never prescribed in isolation. It is one tool within a comprehensive metabolic strategy that includes Lifestyle Medicine foundations, hormone optimization where indicated, and ongoing physician oversight through your Wellness Program. The goal is not just weight loss; it is metabolic resilience.

  • GLP-1 medications are not appropriate for everyone. You are likely not a candidate if any of the following apply:

    • Personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2)

    • History of pancreatitis or significant pancreatic disease

    • Severe gastrointestinal conditions such as gastroparesis

    • Current pregnancy or active plans to become pregnant

    • Current breastfeeding

    • Known allergy or hypersensitivity to semaglutide, tirzepatide, or any components of these medications

    • Type 1 diabetes or history of diabetic ketoacidosis

    Additionally, GLP-1 therapy requires careful consideration and monitoring in women with a history of eating disorders, significant kidney or liver disease, or those on certain diabetes medications. We review all relevant medical history during your EMBARK evaluation before any recommendation is made.

    If GLP-1 therapy is not appropriate for you, Path Health offers alternative metabolic strategies. The right plan is built around your clinical picture, not around a single tool.

  • This is one of the most important and most honest questions to ask. The short answer: for many women, stopping GLP-1 medication without a strong Lifestyle Medicine foundation in place does lead to weight regain. The research is consistent on this point.

    This is exactly why Path Health does not prescribe GLP-1 therapy in isolation. The time while a client is on GLP-1 medication is used actively to build the nutritional habits, the muscle mass, the metabolic resilience, and the hormonal balance that support lasting, sustainable results after the medication is eventually tapered or discontinued.

    The goal is not to be on GLP-1 therapy indefinitely, although there is no shame in this being a long-term therapy for the treatment of obesity. Either way, the ultimate goal is to use it strategically as a tool that creates the conditions for a sustainable metabolic shift, one supported by the broader clinical plan Path Health builds with you.

  • Hormones and metabolism are deeply interconnected, and this connection is often the missing piece in conventional weight loss approaches.

    Estrogen plays a direct role in insulin sensitivity, fat distribution, and lean muscle preservation. When estrogen declines in perimenopause and menopause, visceral fat accumulates more readily, insulin resistance increases, and the body's metabolic rate slows. Testosterone, even at the lower levels present in women, supports muscle synthesis and fat oxidation. Thyroid hormones regulate the overall metabolic rate.   • Yes. Path Health offers microdosing of GLP-1 medications for appropriate candidates. It is important to understand upfront that microdosing GLP-1s for the purposes described below is not FDA-approved and represents an off-label use of these medications. We will discuss this distinction with you in full during your consultation so you can make a fully informed decision.

    Microdosing refers to the use of GLP-1 receptor agonists at sub-therapeutic doses significantly lower than those used for weight loss, to achieve targeted physiologic benefits without the appetite suppression and weight reduction effects of standard dosing. This approach is most relevant for individuals who are not clinically overweight or obese but who are seeking optimization of specific health markers.

    Emerging evidence and clinical experience suggest that microdosing GLP-1 medications may support:

    • Reduction of systemic inflammation:  GLP-1 receptors are present throughout the body, including in immune and cardiovascular tissue, and low-dose activation may help modulate inflammatory pathways

    • Optimization of metabolic markers:  including insulin sensitivity, fasting glucose, lipid profiles, and other cardiometabolic indicators in individuals who fall within a normal weight range but whose labs reflect early metabolic dysfunction

    • Longevity and preventive health goals:  for clients focused on long-term cardiometabolic health rather than acute weight loss

    Candidacy for microdosing is determined through your EMBARK evaluation. It is not appropriate for everyone, and we will only recommend it when your clinical picture, goals, and risk profile support its use.
